地域圏統合型医療情報データベースを用いた抗菌薬適正使用の評価に関する研究 Evaluation of antimicrobial stewardship using regional medical information database network 田辺, 正樹 Tanabe, Masaki 高橋, 佳紀 Takahashi, Yoshinori 抗菌薬適正使用 薬剤耐性 地域連携 データベース application/pdf 薬剤耐性（AMR）対策として（１）抗菌薬適正使用支援チーム（AST）による各種活動が抗菌薬使用量や薬剤耐性菌の発生状況などに与える影響と（２）三重地域医療圏統合型医療情報データベース（Mie-LIP DB）を用いた抗菌薬適正使用の評価を行った。（１）三重大学病院において2018年8月からのAST活動により、抗緑膿菌薬は約15%減少（カルバペネム系は約50%減少）し、カルバペネム耐性緑膿菌の分離頻度、Clostridioides difficileの発生率も低下傾向を認めた。（２）Mie-LIP DBを用い、三重県内９病院の肺炎・尿路感染・上気道感染の抗菌薬処方人数の比較検討が可能であった。 The following two measures against antimicrobial resistance (AMR) were evaluated: (1) the impact of various activities by the Antimicrobial Stewardship Team (AST) on antimicrobial use and the occurrence of drug-resistant bacteria, and (2) the appropriate use of antimicrobials using the Mie Local Integrated Healthcare Information Database (Mie-LIP DB). (1) AST activities at Mie University Hospital since August 2018 have resulted in an approximately 15% decrease in anti-Pseudomonas drugs (approximately 50% decrease for carbapenems), and a downward trend in the frequency of carbapenem-resistant Pseudomonas isolates and the incidence of Clostridioides difficile was also observed.
