Subsequent studies have demonstrated that IgA is the predominant immunoglobulin in various external secretioms. Tomasi & Bienenstock showed that IgA present in the external secretions was different in che mical and immunological properties from IgA of serum and produced locally.Moreover, studies of the secretory immune response following virus infection have shown that the titer of virus nutralizing antiboddies closely parallels concentration of IgA in nasal washing. Secreyory IgA may act as an initial immunologic barrier to the portal of entry of virus infection. Recentory, it was also demonstrated that IgG ,IgM and IgE were produced localy. These local immunoglobulins may play an important role against viral and bacterial infections in the human respiratory and other mucous surfaces. Chronic sinusitis is one of the most prevalent nasal illnesses in Japan. Conservative medical treatments i.e.medical therapy and/or sinus irrigation is of little value in some of these cases. Even surgical treatment is not always satisfactory. The pathogenesis of chronic sinusitis has not been fully understood yet. During the past decade, a considerable number of investigations have accumulated concerning local immune defense mechanisms and some investigators have shown that immunoglobulin deficiencies result in sinobronchial infection.
