{"created":"2023-06-19T11:37:17.444054+00:00","id":4536,"links":{},"metadata":{"_buckets":{"deposit":"3b19369e-9056-462e-8ac2-7220a70085ec"},"_deposit":{"created_by":13,"id":"4536","owners":[13],"pid":{"revision_id":0,"type":"depid","value":"4536"},"status":"published"},"_oai":{"id":"oai:mie-u.repo.nii.ac.jp:00004536","sets":["334:335:354:356"]},"author_link":["9070","9071"],"control_number":"4536","item_4_biblio_info_6":{"attribute_name":"bibliographic_information","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2007-03-20","bibliographicIssueDateType":"Issued"},"bibliographicPageEnd":"95","bibliographicPageStart":"89","bibliographicVolumeNumber":"9","bibliographic_titles":[{"bibliographic_title":"三重看護学誌"}]}]},"item_4_description_14":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_4_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"本研究の目的は, 教育における倫理的配慮のあり方を研究する一環として, 看護師が学生時代に体験した学内における基礎看護技術演習を遡及的に調査することである. 本研究では, その演習方法や内容を明らかにするとともに技術演習に対する看護師としての意見を分析した. 認定看護師研修受講の全国の看護師 190 名を対象に, 自記式質問紙調査を実施した. 質問は, 特別に倫理的配慮を要する基礎看護技術 7 項目に焦点をあてた. 実習方法を尋ねたところ, 身体的侵襲の加わる 5 つの技術項目のうち, 静脈内採血が 「学生同士で実施」 を体験した割合が最も高かった (80%). 注射は約半数が 「学生同士で実施」 を体験していた. そして, 約 10%の看護師は採血も注射も, 演習において学生に対しても模型等に対しても実施した体験がなかった. プライバシーを侵害する可能性のある技術項目では, 全身清拭は約 86%が学生同士で実施体験し, 床上排泄は実施・非実施がほぼ同率となっていた. 看護師である現在の立場での, 演習に対する感想の中で, 採血及び注射については, 学生時代に実施した者も実施しなかった者も, 「実施し体験することが妥当」 とする回答が多かった. 看護技術実施体験の是非については, 「肯定的」 が 4 割を越え, 「否定的」 意見はなかった. 患者体験の必要性については, 「肯定的」 と 「否定的」 意見に分かれた. \n 以上の結果は, 技術修得は看護教育にとって必須の到達目標であることを改めて浮き彫りにした. しかし, 教育に携わるものは, 身体侵襲性や人権侵害性という倫理的観点から, 学生の権利を擁護しつつ教育効果をあげるために, 最低限でも模型を利用した実施体験と反復練習が必要である. それとともに, 患者の気持ちを理解するために, 「演習での患者体験」 ではなく, 体験がなくても共感できる観察力と想像力を育むことが必要である.","subitem_description_language":"ja","subitem_description_type":"Abstract"},{"subitem_description":"The aim of this study was to examine the ethical aspects of nursing education using a retrospective survey of nurses experiences with in-class practice for basic nursing skills as a nursing student. We analyzed nurses perspectives on in-class practice for nursing skills and delineated the methods and contents of such education. Using self-administered questionnaires, we conducted a survey of 190 nurses nation-wide who attended the Certified Expert Nurse (CEN) training course. The instrument focused on 7 basic nursing skills that necessitate special ethical considerations. The results of the survey showed that among the 5 skills that involve invasive procedures, intravenous blood collection ranked highest with 80% of respondents having practiced on fellow students. As for injection, approximately half of respondents had practiced on fellow students. About 10% of nurses had never drawn blood nor given an injection, neither to a fellow student nor to a dummy, during their in-class training. Regarding skills that could infringe on personal privacy, approximately 86% of respondents had performed a full-body bed bath on a fellow student, and about the same percentage had performed in-bed toileting as those who had not. When asked about how they now reflect on their in-class practice, both those who had practiced and who had not practiced drawing blood and giving injections answered that, it is appropriate to have some pragmatic experience with nursing skills. Regarding the pros and cons of practicing nursing skills, over 40% of nurses responded that it was positive; no respondents answered negative. However, regarding the need to acquire personal experience as a patient, respondents answers were divided between positive and negative. These findings highlight that training for nursing skills is an indispensable objective of nursing education. However, educators need to consider the ethical risks of invasive procedures and potential human right violations, and thus should teach by rote and through using a dummy in order to increase the effectiveness of education while still protecting students human rights.
