| 内容記述 | 〈研究目的〉 本研究では，心神耗弱・喪失状態において重大な他害行為を行い医療観察法による入院となった患者が，対象行為や精神疾患，長期にわたる指定入院医療機関での入院について，どのような体験をして，その体験をどのように意味付けているのかを記述し，理解することを目的とする． \n〈研究方法及び分析方法〉 医療観察法入院患者1名を対象に半構成的面接を行った．そして，面接での患者の語りをハイデガーの存在論に基づくBennerの解釈的現象学の帰納的質的研究方法により分析した． \n〈結果・考察〉 1．医療観察法入院患者の対象行為の意味は，他者に対しての他害行為そのものがストレスの対処であり，他者の気持ちに関心を向けることができず，他害行為を行っていた． 2．他害行為時，医療観察法入院患者はストレスを抱えて苦しんでおり，他のことに関心を向けることのできないような状況になり他害行為を行っていた． 3．医療観察法入院患者は，医療観察法入院後，ストレスに対して病気としての意味を持ち，普段の入院生活の中で病気への対処を実践していた． 4．医療観察法入院患者は，他害行為時は被害者の気持ちに関心を向けることができない存在であった．しかし，入院中に，スタッフによる「気遣い」を受け，大事に思われていと感じることで，他者の気持ちに関心を向けることができるようになり，他者を「気遣う」ことができる存在となっていた． 5．医療観察法入院患者の語りから，他害行為時どのような苦しみがあったのかや，普段の入院生活での行動の意味を患者自身の世界で意味付けするままに捉え，理解していくことの必要性が示唆された． |

| 内容記述 | The purpose of this study was a inpatient with the Forensic Mental Service that raised serious violence in feeble-mindedness / a loss state has experience that how is it about the violence and a mental disease, hospitalization and describes the experience how you give a deep significance, and it is what it understands. \n Method and data analysis The subjects were a inpatient with the Forensic Mental Service. The data interviewed by the researcher was analyzed in using a phenomenological approach of the interpretation of Benner based on Heideggers ontology. \n Finding and Discussion 1． As for the meaning of the violence of a inpatient with the Forensic Mental Service, the violence itself for others is measures of the stress. And he did violence without can turn concern to the feeling of the victim. 2． When a inpatient with the Forensic Mental Service did violence, he suffered with stress. And it was in a situation that he cannot turn concern about others, and he did violence. 3． A inpatient with the Forensic Mental Service had a meaning as the disease for stress after the Forensic Mental Service was hospitalized. And he practiced the measures to a disease in everyday hospitalization life. 4． When a inpatient with the Forensic Mental Service did violence, he was the existence that cannot turn concern to the feeling of the victim. However, he took care by the staff while his hospitalization. And he gets possible to turn concern to the feeling of others by feeling that he is taken good care of. And He got possible to care with others. 5． It was suggested that it was necessary what a nurse thought about and to understand by the narrative of a inpatient with the Forensic Mental Service as he gave a deep significance and he had what kind of pains when he did violence and did the meaning of the action by his everyday hospitalization life. |
