| 内容記述 | 本研究の目的は， 本学における在宅看護実習の形態の違いによる， 学生の学びの変化を明らかにすることから， よりよい在宅実習教育のあり方を検討していくことである． 本学で在宅看護実習を履修した 4 年次生のうち， 調査に同意が得られた 2004 年度 51 名， 2005 年度 73 名の計 124 名に実習終了時に行ったアンケート調査の結果を統計的に比較して考察した． 1 ． 2004 年度は， 主に近親者を対象に健康課題を抽出し， 看護過程を展開する形態であり， 祖父母を選択する学生が多くみられた． ここでの学びでは， 〈生活への適応状態を知るとともに， 障害となっている要因を理解する〉の項目での理解度が高くみられた． 2 ． 2005 年度は， 訪問看護ステーションでの実習を必須にした． と同時に自ら実習方法を選択し， 訪問看護事業所と交渉し， 実習を進めるという Self-directed Learning （SDL） の学習方法を取り入れた． ここでの学びは， 〈在宅看護の現状を理解する〉〈在宅看護に必要な行政機関・保健・医療・福祉機関および他職種間の連携や協働活動を理解する〉〈施設内看護と在宅療養時の利用者がおかれている立場の違いを理解する〉〈在宅看護実習を通して自己の看護観を見つめ直す〉の項目での理解度が高く見られた． 3 ． 訪問看護事業所での実習には一人の訪問看護師から指導を受けることが多くなるという現状があるため， 多面的に幅広く体験的に学ぶためには， 個別の指導や実習報告会などの学内における指導方法の確立と， 訪問看護事業所との協力体制を構築していけるようなさらなる工夫が必要である． |

| 内容記述 | The objective of the present study was to elucidate changes in learning among students due to differences in the type of practical training in home nursing at our university, in order to identify more effective methods of practical training in home nursing. A statistical comparison was conducted on the results of questionnaires administered at the end of practical training to a total of 124 fourth-year students, specifically 51 students in 2004 and 73 students in 2005, who completed practical training in home nursing at our university and consented to participate in the survey. The following results were obtained: 1） In the training method used in 2004, students were instructed to apply nursing processes after identifying health problems primarily among close relatives. Many students selected their grandparents as subjects. This method resulted in a high level of understanding in the item of understanding the causes of impairment while determining the state of adaptation to life. 2） The training method used in 2005 required students to participate in practical training at visiting nurse stations. Training involved self-directed learning （SDL）, a learning method in which students engage in practical training by independently selecting methods of practical training and negotiating with visiting nurse stations. This method resulted in high levels of understanding in the following items: understanding the current state of home nursing, understanding the cooperation and collaborative activities between government agencies, health, medical, and welfare organizations, and other professions that are necessary for home nursing, understanding differences in the situations of users of facility nursing and home care, and reviewing personal nursing philosophy through practical training in home nursing. 3） Since practical training at visiting nurse stations often involves provision of guidance from a single visiting nurse, further improvements such as establishment of guidance methods such as individualized guidance and sessions for reports of practical training within the class, as well as development of a system for cooperating with visiting nurse stations are necessary for enabling a more multidimensional, extensive, and experiential method of learning. |
