本研究では、大腸癌術前CT画像を解析し、腰椎L3下縁レベルにおける腸腰筋・多裂筋を評価し、Psoas Mass Index (PMI:腸腰筋面積/身長2)・Intramuscular Adipose Tissue Content (IMAC:多裂筋CT値/皮下脂肪CT値)・Modified IMAC(mIMAC:多裂筋CT値-皮下脂肪CT値)を計算し、その臨床的意義や腫瘍学的予後に与える影響を解析し、術前PMI低下やmIMAC低下が有意な予後不良因子となることをあきらかにした。また遠隔転移組織分泌型miRNAが骨格筋量低下を引きおこすあらたな機序を解明した。 |

We evaluated psoas mass index (PMI) or modified intramusclular adipose tissue content (mIMAC) using preoperative computed tomography, and revealed that preoperative myopenia or myosteatosis was an independent prognostic factor in colorectal cancer. In addition, assessment of miRNA expression using clinical specimens and series of in vitro experiments demonstrated that circulating miRNAs secreted from distant metastasis could induce myopenia in colorectal cancer patients. |
