| 内容記述 | この研究の目的は，外来通院中の進行肺がん患者のがんに対するストレス－コーピングとソーシャル・サポートの実際を明らかにすることである． 対象は，外来通院中の進行肺がん患者13名で，がんによるストレスとその対処について半構成的面接を行い，ソーシャル・サポートの視点で質的に分析した． 外来通院中の進行肺がん患者のストレス－コーピングとして，【がんや治療による生活の支障への対処】【がん治療への主体的な取り組みによる対処】【がんによる精神的不安への対処】【がんによるネガティブな感情への対処】【自己価値のゆらぎへの対処】の5つの大カテゴリーが抽出された． 患者のすべてのコーピングにサポートは密接に関わっていたが，サポートが不十分または非効果的な場合，患者が対処に苦しんでいる状況も明らかとなった．看護師は，外来通院中の進行肺がん患者に対し，効果的なコーピングの強化ができるよう，サポートを行う必要がある．また患者は周囲への遠慮や我慢などから，かえってストレスを増大させる危険性もあるため，周囲の人々に働きかけて負担が少なく生活を維持できるようサポートを行う必要がある．特に「自己価値のゆらぎ」は，がんや治療だけでなく，周囲の環境によってももたらされるストレスであるため，注意深いかかわりが必要である． |

| 内容記述 | The purpose of this study was to clarify the stress-coping of outpatients with advanced lung cancer and the social support related to the stress-coping. The subjects were 13 outpatients, and the data was collected through the interviews about what kinds of social supports and stress-coping they had. As the results of qualitative analysis, there were five large categories:【Coping to the life hindrance with cancer and treatment】【Coping to the treatment by doing an independent action】【Coping to the uneasiness of cancer】【Coping to the negative feelings with cancer】【Coping to the fluctuation of the self-value】. The outpatients with advanced lung cancer coped their stress while receiving the social support of their family and neighboring people. But it was revealed that they increased their stress with their restraint and patience to the family and neighborhood. This study suggests that the nurses can support the outpatients in listening their talk and educating the effective coping as self care. And also, there is a need of the careful relationship between patients and nurses, because the fluctuation of the self-value is the stress caused by not only cancer and treatment but also the neighboring environment. | |||||||||||||||||
