| 内容記述 | 本研究の目的は， 外来化学療法を受ける根治困難な非小細胞肺がん患者が抱く苦痛の様相を明らかにし， 看護支援のあり方を明らかにすることであった． 外来化学療法を継続して受けており， 自らの病状を理解している 8 名の非小細胞肺がん患者を対象に日頃感じている苦痛について半構成的面接調査を実施した． 得られたデータは質的帰納的に分析をした． その結果， 外来化学療法を受ける非小細胞肺がん患者の苦痛は， 【化学療法治療に関連した苦痛】【がんとの共存から生じる苦痛】【外来通院治療に関連した苦痛】【役割の変調に関連した苦痛】【人間関係に関連した苦痛】【スピリチュアルな苦痛】の 6 つのカテゴリーに分類された． これらの結果から， 外来化学療法を受ける肺がん患者の苦痛には次のような特徴があると考えられた． ①身体的な苦痛の中心は， 倦怠感・末梢神経障害・脱毛などの副作用症状であり， それらは他に抽出された苦痛と関連していた． ②効果のある限り治療を継続するという闘病生活において， 慢性的な精神的苦痛が存在していた． ③外来化学療法という形態が， 在宅期間の延長や社会生活の継続を可能としている反面， 実際には生活パターンや人間関係の変調などがみられ， 苦痛を 1 人で抱え込む傾向があった． 以上のことから， 看護師は外来化学療法を受ける患者の苦痛の特徴を念頭に置いて， 治療を妨げる要因を早期に察知した予測的な看護介入や， 副作用の出現などによる日常生活への影響に対するケア， およびタイムリーに患者の悩みの相談に応じていける継続的な外来看護の支援体制を確立する必要性が示唆された． |

| 内容記述 | he purpose of this study was to clarify the aspect of the pain related with non-small cell lung cancer patients receiving outpatient chemotherapy held, and to consider nursing care. Data were collected through semi-structured interviews for eight patients who knew their own diseases and the necessity of outpatient chemotherapy. Inductive and qualitative analysis were employed for analyzing data. Six categories were identified.【pain related to chemotherapy】,【pain caused from the existence of cancer itself】,【pain related to outpatient chemotherapy】,【pain related to the modulation of the role.】,【pain related to human relations】,【spiritual pain】. From the obtained results, there were the following features in the pain of lung cancer patients receiving outpatient chemotherapy. ① The main physical pain consisted of side effects such as fatigue, peripheral neuropathy, and depilation. ② There was the emotional pain in an uncertain prognosis and continuing treatment without limitation. ③ The outpatient chemotherapy enabled the extension of staying home and the continuity of the social life. But a certain pain existed always in the patients because the life pattern and the interpersonal relationship had to be changed by the chemotherapy. Therefore, nurses need to understand about the feature of the pain related with patient receiving outpatient chemotherapy and to establish the support system for the outpatients such as consultation of their problems, pain, and education for serf-care management at their home. |
