| 内容記述 | 小児副鼻腔炎に対しては保存的療法が最も適応となる。副鼻腔洗浄や抗生物質とステロイド剤のエアロゾルによる投与が一般的に行われる。このような保存的療法により改善を認めない場合、１０歳以上の小児では手術療法の適応となる。鼻茸も手術的に除去されねばならない。１６歳以上の患者においては、中等度や高度副鼻腔病変は手術的に治療される。上顎洞にのみ中等度の病変を認める場合には手術療法は３カ月間の保存的療法が有効でない場合のみ適応となる。このような例では上顎洞の繊毛上皮の形態が良好に保たれているため、鼻内上顎洞手術が適応となる。高齢者では保存的または手術的療法は患者の全身状態とQOLを考慮して決定されるべきである。 | |||||||||||||

| 内容記述 | Medical management is the most common modality of treatments for pediatric sinusitis. Sinus lavage and administration of antibiotics and/or steroids in a form of aerosols are usually employed. Failure of these medical managements is an indication for surgical treatment in the patents more than 10 years old. Nasal polyps must be treated surgically. In the patients more than 16 years old, severe and moderate sinus lesions are treated by surgery. If a patient shows, moderate lesion localized in the maxillary sinus, surgery is indicated only following a failure of adequate medical managements for 3 months. In such cases, the functional sinus surgery is indicated, because ciliated epithelium of the maxillary sinus is well preserved. Medical management is absolutely indicated to the patients with mild sinus lesion. In aged patients, medical or surgical management of sinusitis must be selected by considering their general conditions and quality of life(QOL). | |||||||||||||
