| 内容記述 | 【目的】外来化学療法を受ける患者の苦痛・困難について看護師と患者の認知を比較し， 外来化学療法看護について検討することを目的とした． 【結果及び考察】外来化学療法患者の苦痛・困難は， 看護師・患者共に【身体面に関する苦痛・困難】【精神面に関する苦痛・困難】【社会面に関する苦痛・困難】【人とのかかわりに関する苦痛・困難】【治療環境に関する苦痛・困難】の 5 つの大カテゴリーに集約された． 身体面では， 治療に関係するものと病状進行による症状に関するものが見出された． 精神面では治療の継続に関するものとがんという病と向き合う苦痛困難， 社会面では時間的なもの， 日常生活に関するもの， 社会活動に関するもの， 人との関係では医療者， 家族， 友人との関係によるもの， が見出された． 看護師と患者の比較における相違点として， 患者の認知は主観的で， 身体面を中心に精神面， 治療環境面， 社会面， 人間関係面， さらにスピリチュアル面が加わり， 各カテゴリーの内容が複雑に絡み合い影響した全人的なものとして存在していた． しかし， 看護師の認知は各カテゴリーの内容の繋がりがなく， 各カテゴリーの内容独自のものと捉えている傾向があった． また， 看護師の認知はなんとなく気づいているが， 患者の体験している苦痛の認識まで至らず， 「気づき」 を 「看護ケア」 に生かすことが出来ない状況であることが示唆された． 今後， 看護師の 「気づき」 を 「看護ケア」 に繋げるため①患者と看護師の認知のズレを意識した看護ケアの提供， ②患者ニーズに対応する外来看護体制の構築， ③がん看護専門看護師の活用によるケアの質の保証の 3 点の取り組みが必要であると考える． |

| 内容記述 | The purpose was to clarify the comparison between nurses' and patients' recognition relating to outpatient's distress and difficulty in receiving chemotherapy and to find out effective nursing care. Ten nurses and 6 patients who were involved in chemotherapy at outpatient clinic were interviewed. And the following results were obtained through qualitative analysis. The patient's distress and difficulty were classified into five categories for both nurses and patients. 1. Distress and difficulty in physical condition: relating to medical treatment and clinical symptom. 2. Distress and difficulty in psychological condition: relating to the duration of the treatment and mind facing to the cancer. 3. Distress and difficulty in socio-cultural condition: relating to daily life and social activity. 4. Distress and difficulty in human relationship: relationships among health care giver, family and friends. 5. Distress and difficulty in treatment surroundings: relating to the surrounding of outpatients department. There was a difference in recognition of those distress and difficulty between nurses and patients. Nurses were inclined to take each category as a separate content, while patients were more apt to take whole categories related in all. Nurses might notice patients' distress and difficulty, but not get in understanding deep enough what patients were feeling. For better future care, nurses need to recognize patients' hardship more to establish more quality-effective nursing care system, and to organize a nursing team including clinical oncology specialists. |
